Registration form
The registration form for joining Venturing Crew 55 is the same form as used for joining a Troop or other BSA program.
The form is a three-part pressure-sensitive document (writing on the top page automatically copies to the next two pages).
To obtain this form, please contact James Cauble, the Crew Advisor.
When complete, the form should be returned to James Cauble, not mailed to the Scout Office.
Medical form
The medical form used by the Venturing program is the same form as used by other BSA programs.

This form needs to be updated annually.
If you have submitted a medical form to Troop 55, you do not need to submit a separate form to Crew 55
as long as the form is still current.
